Maintenance Request

Use this form to provide your customers the ability to request maintenance service or support from your team. Main list is configured to collect all information needed to start the maintenance call to either schedule a visit or to answer some questions. The customer can even add pictures to their request to provide more detail. These pictures are stored automatically in the Request Images library.

Modify this templates to better fit your needs by re-phrasing the questions or adding your own. Form can also be used externally for fully anonymous submissions.
